Appointment Preparation
When you arrive for your appointment, you will be asked to sign an Authorization to Collect and Disclose Information and Consent Form. By signing this form, you agree to and authorize ECS Safety Services Ltd. to collect your personal and health information through the assessments and testing required by your Employer in accordance with applicable health information and privacy legislation. You acknowledge the personal and health information collected will only be disclosed to your Employer and to you. You also consent to the performance of the assessments and testing.
